According to the Houston Police Department, FBI SWAT made entry into the home. One team was in the rear and one in front. The rear team went to the window the hostage was at with no lights. As they were breaching the window the breaching tool fell in. The agent then reverted to using his M4 weapon as he was, someone grabbed the weapon from the inside.
After a struggle and not knowing who was trying to take the weapon, the agent fired. Once they made entry they found it was the hostage that was grabbing the weapon. He was immediately transported to LBJ Hospital where he died.
