Carrie Turner Ballard age 83, was last seen at approximately 11:00 a.m. Monday morning at an assisted living center in the 32000 block of Wright Rd. near Deckers Prairie. Ballard suffers from Dementia and was last seen wearing a blue long sleeved turtle neck sweater and blue jeans. She was possibly carrying a white pillow case. She may be wearing a necklace with a tag bearing her personal identifiers and the identity of the facility from which she departed.
